The conference experience (as well as many other activities) will be captured in photographs, video and audio. Planetshakers reserves the right to use the material for promotional purposes. Images of people attending or participating in these activities may be used in our TV ministry, resources, and for other ministry or promotional purposes. I understand it is a condition of all registrants’ entry into the conference that all claims are waived in relation to inclusion of their likeness in such films or recordings. If I am a parent/guardian, I give my permission for my child/ward to be photographed or videotaped. I understand that the image may be displayed in the church publications, church buildings or websites. I understand that as a precaution my child’s/ward’s name will not be published or linked with photographs.

Permission to Participate

1. As parent/guardian of the Attendee, we give permission for the Attendee to participate in Planet Boom held at the Citipointe Church Carindale/Calvary Family Church.

Activities

2. We acknowledge that activities will be on offer for Attendees during Planet Boom, and the Church will provide adequate supervision for those activities.

3. We acknowledge that some of the activities may at times be hazardous. These activities include:
a. Activities/games as per the Planet Boom itinerary.
b. Transport (Bus/Car) to and from the venue as requested by participants.

4. We agree that the Attendee participates in any activity at their own risk and the Church shall not be liable for any personal injury, death, loss or damage occasioned whether such liability arises at common law or in any other way.

5.We acknowledge that:
a. The Church will take all reasonable precautions within its power to minimize exposure to known risks, hazards and dangers associated with any Church facilitated activity that is undertaken by Attendees during Planet Boom that can be reasonably foreseen.
b. Attendees are not required to participate in any activities facilitated by the Church, and attendees participate in any activities facilitated by the Church during Planet Boom at their own risk.
c. During Planet Boom, all attendees must follow the lawful and reasonable directions of the Church and its representatives at all times.
d. We must advise the Church of any medical conditions or impairments that may affect the Attendee’s ability to safely participate in any of the activities during the Planet Boom.

Indemnity

10. We indemnify the Church against any loss or damage to property, equipment or personal effects belonging to the Attendee, or any other person arising either directly or indirectly from or in connection with Planet Boom.

11. We release and further agree to indemnify, hold harmless or reimburse the Church or the individual members, agents, employees and representatives thereof,  from and against, any claim which the Attendee, any parent or guardian, any sibling, or any other person, firm or corporation may have or claim to have, known or unknown, directly or indirectly, for any losses, damages, injuries, illness, accidents, and lack of competent medical care arising out of, during, or in connection with the attendee’s participation in Planet Boom(including all forms of transportation/travel) or the rendering of emergency medical procedures or treatment, if any.

Medical Treatment

12. We warrant that we have provided the Church with any medical information relevant to the Attendee prior to the commencement of Planet Boom.

13. In case of an emergency, we consent to the Attendee receiving any medical care deemed necessary by a representative of the Church (under the advice of local medical practitioners). This may include, but is not limited to, travel by ambulance and hospital treatment. The attendee consents to an appointed first aid officer of the Church to give basic first aid care if required. We agree to pay all medical expenses that may be incurred.
 

Other

14. We acknowledge that we have read, understood, and will comply with the terms of this waiver, all relevant policies and expectations of the Church regarding the Attendee’s participating in the Camp attendance at Planet Boom.

As the Parent/Carer/Guardian, I have read and understood the above waiver for my child to attend PlanetBoom 2026.
