Calvary Family Church ACN 080 375 793 (the “Church”)
Permission to Participate
- As parent/guardian of the Attendee, we give permission for the Attendee to participate in the Calvary Family Church Youth Camp (the camp) held at MTCC 237 Beacon Rd, Tambourine Mountain Qld 4272.
Activities
- We acknowledge that activities will be on offer for Attendees during Youth Camp and the Church will provide adequate supervision for those activities.
- We acknowledge that some of the activities may at times be hazardous. These activities include:
- Transport from Calvary Family Church to the venue and return;
- Other activities/games as per the Youth Camp program;
- Swimming at the Tower 45 – Seaway (The Spit Beach). Swimming will be at a patrolled beach;
- Swimming at Cedar Creek Falls – Tambourine Mountain. Swimming will be included under the supervision of a current Bronze Medallion holder.
- Firepit at the camping venue.
- Bush Walking – Multiple Tambourine Mountain Locations.
- We agree that the Attendee participates in any activity at their own risk and the Church shall not be liable for any personal injury, death, loss or damage occasioned whether such liability arises at common law or in any other way.
- We acknowledge that:

- The Church will take all reasonable precautions within its power to minimize exposure to known risks, hazards and dangers associated with any Church facilitated activity that is undertaken by Attendees during the cam that can be reasonably foreseen.
- Attendees are not required to participate in any activities facilitated by the Church, and attendees participate in any activities facilitated by the Church during the Youth Camp at their own risk.
- During the camp, all attendees must follow the lawful and reasonable directions of the Church and its representatives at all times.
- We must advise the Church of any medical conditions or impairments that may affect the Attendee’s ability to safely participate in any of the activities during the camp.
Food and Accommodation
- The Church will be responsible for providing food for the Attendee, and holds a valid food handling certificate.
- We warrant that we have notified the Church of any food allergies suffered by the Attendee prior to the commencement of the camp of their allergies. We acknowledge that the Church will practice the appropriate food handling protocols, however, cannot guarantee that the meals provided will be free from allergens. Due to the methods and surfaces in which foods may be cooked there is no guarantee that traces of the food an attendee may be allergic to will not be present in the food in which the attendee may be served.
- The Church will provide accommodation for Attendee.
- We acknowledge that the Church does not provide insurance for the Attendee’s personal equipment, such as mobile phones, laptops, bags, wallets and keys.
Indemnity
- We indemnify the Church against any loss or damage to property, equipment or personal effects belonging to the Attendee, or any other person arising either directly or indirectly from or in connection with the Youth Camp.
- We release and further agree to indemnify, hold harmless or reimburse the Church or the individual members, agents, employees and representatives thereof, from and against, any claim which the Attendee, any parent or guardian, any sibling, or any other person, firm or corporation may have or claim to have, known or unknown, directly or indirectly, for any losses, damages, injuries, illness, accidents, and lack of competent medical care arising out of, during, or in connection with the attendee’s participation in the camp (including all forms of transportation/travel) or the rendering of emergency medical procedures or treatment, if any.
Medical Treatment
- We warrant that we have provided the Church with any medical information relevant to the Attendee prior to the commencement of the Youth Camp.
- In case of an emergency, we consent to the Attendee receiving any medical care deemed necessary by a representative of the Church (under the advice of local medical practitioners). This may include, but is not limited to, travel by ambulance and hospital treatment. The attendee consents to an appointed first aid officer of the Church to give basic first aid care if required. We agree to pay all medical expenses that may be incurred.
Other
- We acknowledge that we have read, understood, and will comply with the terms of this waiver, all relevant policies and expectations of the Church regarding the Attendee’s participating in the Camp attendance at the camp.
As the Parent/Carer/Guardian, I have read and understood the above waiver for my child to attend Ignite Retreat 2024.
